The Arthrex TPLO rotation chart is a surgical planning reference used during tibial plateau leveling osteotomy (TPLO) surgery.

It tells the surgeon exactly how many degrees to rotate the tibial bone segment to reach the target tibial plateau angle for that specific dog.

Without it, the surgeon would need to calculate rotation manually for each case a slower process with more room for error.

What the Arthrex TPLO rotation chart does

TPLO surgery works by rotating the tibial plateau the top surface of the tibia to a shallower angle so the stifle joint is mechanically stable without a functioning cranial cruciate ligament (CCL).

The rotation chart shows the degrees of rotation needed to achieve a target tibial plateau angle based on the preoperative measurement. This chart helps surgeons plan and execute the bone cut and rotation precisely. Using this chart reduces guesswork and improves the accuracy of the surgical correction.

The chart is a two-variable lookup tool. The input is the measured preoperative TPA. The output is the degrees of rotation to apply to the osteotomized bone segment.

The target postoperative TPA for most TPLO procedures is approximately 5 degrees.

The TPLO method involves performing a quarter-circle osteotomy of the proximal tibia and rotating the osteotomized fragment to adjust the tibial plateau angle. The desired postoperative TPA can be achieved by calculating the necessary rotation angle from the preoperative TPA, typically around 6.5 degrees on average.

How the preoperative TPA is measured

This angle indicates how sloped the tibial plateau is, which influences the degree of rotation needed. Accurate measurement is essential for the chart to provide correct rotation values.

TPA is measured on a properly positioned mediolateral radiograph of the tibia.

The radiograph must be taken with the stifle in a standardized position typically with the joint at approximately 135 degrees of extension.

Two reference lines are drawn: one from the femoral condyle along the tibial plateau, and one from the intercondylar eminence to the center of the talus.

The angle between these two lines is the TPA.

A correctly positioned radiograph is essential. Patient rotation during the radiograph shifts the apparent TPA and produces inaccurate chart values, leading to imprecise surgical rotation.

Normal dogs have a TPA of approximately 18 to 25 degrees. Dogs with steeper tibial plateau angles require more rotation. Dogs with lower angles require less.

Using the chart intraoperatively

During surgery, after the curved osteotomy cut is made in the proximal tibia, the bone segment is rotated using the Arthrex jig system.

The surgeon uses the chart value determined during preoperative planning to set the jig to the correct rotational position before applying the plate.

The TPLO rotation chart serves as a visual aid, ensuring accuracy and precision during surgery. With its implementation, veterinarians can enhance surgical accuracy, minimize errors, and improve overall patient outcomes.

Post-rotation, the plate is applied and the position is confirmed with intraoperative radiographs before final screw fixation.

Consequences of incorrect rotation

Under-rotation (insufficient correction) leaves a TPA above 5 degrees. The stifle remains mechanically susceptible to cranial tibial thrust, potentially leading to ongoing instability, pain, and accelerated OA progression.

Over-rotation produces a TPA below 0 degrees (caudal slope). This reverses the biomechanical problem now the tibia is pushed caudally during weight-bearing, creating abnormal joint mechanics and potential implant stress.

Accurate rotation of the tibial plateau is critical to restore normal joint mechanics after TPLO surgery. The rotation chart helps surgeons avoid under- or over-rotation, which can lead to poor outcomes or complications.

The Arthrex system context

The rotation chart is designed specifically for use with Arthrex TPLO implants, jigs, and equipment. Arthrex is one of the two most widely used TPLO implant manufacturers in veterinary orthopedics alongside Synthes.

During TPLO surgery, the surgeon makes a curved cut in the tibia and rotates the bone segment to change the slope of the tibial plateau. The Arthrex TPLO plate is then applied to hold the bone in its new position.

Frequently asked questions

Is the Arthrex TPLO rotation chart the same as a generic TPLO rotation chart?

No. Different TPLO implant systems (Arthrex, Synthes) use different plate designs and jig systems. The rotation chart is calibrated to the specific system's geometry.

An Arthrex rotation chart should only be used with Arthrex instruments and jigs, not with Synthes equipment or other systems.

Can the surgeon skip the chart and calculate rotation manually?

Mathematically it is possible, but the chart standardizes this calculation and reduces intraoperative variability. Most surgeons using the Arthrex system use the chart routinely rather than calculating manually for each case.

A dog with a preoperative TPA of 28 degrees needs more rotation than one with a TPA of 20 degrees. The chart accounts for this individual variation.

Is the rotation chart used for bilateral TPLO cases?

Yes. Each limb is planned independently because TPAs can differ slightly between the left and right tibia. Separate measurements and chart lookups are performed for each leg if both are being operated.
